Family Assessment Form: A Practice-Based Approach to Assessing Family Functioning Book

The Family Assessment Form helps workers to assess families at the beginning of service, to develop service plans, to monitor progress, and to assess outcomes.

Originally designed as a standardized form, it's also a highly adaptable tool for a variety of clinical, procedural, and/or program needs. It uses a nine-point rating scale to enable workers to complete a psychosocial assessment recordable in a quantitative manner that allows for monitoring family progress. The Family Assessment Form is also a valuable research tool and information on reliability and validity are provided.
